Travel to the outlying locations beyond a 35 mile radius of the hospital will be reimbursed according to Section B and shall be in accordance with (IAW) with Section C.11 of the basic contract.  c.  The HCW shall communicate with the Line Commanders to make them aware of the Deployment Health Clinic benefit, to clarify who is eligible to receive services, and to initiate the screening process. Communications will take place at the clinic and “in the field," with the Line units. d.  The HCW shall coordinate with MTF assigned personnel to schedule initial patient screenings. e. Screening. The DHC HCW shall provide deployment health screening services to all service members either individually or in groups, at the clinic or at the Line unit as determined necessary.             (1) The FNP/PA shall perform Deployment Health screenings, assess the results and refer patients for further treatment as indicated. An electronic or web-enabled DD Form 2900 (or Standard Form 600 if electronic or web-enabled access is not available) shall be utilized for post deployment health reassessments, an electronic or web-enabled DD Form 2795 shall be utilized for pre-deployment assessments, and an electronic or web enabled DD Form 2796 shall be utilized for post deployment screenings. f. Administrative.             (1) The HCW shall coordinate with MTF assigned personnel to schedule initial and follow-on appointments or referrals for service members before they leave the clinic or the screening at the Line unit as appropriate.             (2) The HCW shall become proficient with the use of the Navy Environmental Health Center (NEHC) electronic methods of completing the Department of Defense (DD) Forms listed above in paragraph 7. e. (1). The contractor HCW shall use the Standard Form (SF) 600, which is the primary form of documentation for medical encounters, and the (SF) 513, which is the primary consultation form.             (3) The HCW shall ensure proper handling, filing and storing of all completed forms in the patient’s medical record and ensure transmission of all electronic forms to NEHC.             (4) The HCW must document all patient cases and must ensure that patient encounters capture the DHC workload in CHCS/AHLTA using the MEPRS code designated by BUMED, Accounting Guidance, published July 2006 or subsequent revision. e. Treatment.             (1) The contractor HCW shall provide care for eligible patients resulting from the DHC screening in accordance with the Deployment Health Clinic (DHC) guidelines located at http://www.pdhealth.mil/.              (2) The PA/FNP who has a patient that requires an admission to the MTF shall follow MTF specific referral/admission protocols.             (3) The contractor FNP/PA will not be privileged for hospital admissions nor will they have after hours on-call duties. The FNP/PA will not be responsible for obstetrical care of DHC patients.             (4) The FNP/PA assigned to the DHC may be utilized for walk-in or other scheduled appointments in support of other MTF clinics only when all other PDHRA and deployment health commitments have been fulfilled. f. Education. The HCW shall participate in providing educational sessions/briefings for returning Units on deployment related illnesses/conditions and the realities of readjusting to life following deployment. g. Follow-on Care. The HCW may be required to perform case management related services for those patients that need follow on care.             (1) The HCW shall ensure that all service members that require follow-on appointments or referrals will have them before leaving the DHC as appropriate.
